there is an etiquette to dua: You should praise Allah first
Request then you can praise again. Never, never, never stop asking forgiveness from Allah--even if you keep repeating it. It's considered arrogance.

Have the conviction in your heart that you will do your best to never commit the sin again.
Beg Allah to forgive you with all your heart. Then be certain that He has forgiven you. |

all suggestions are good. id add a bit of sincerity and maybe tasbeehs of 'astaghfirullah' to your list.

Shaddad bin Aus (ra) narrated that the Prophet (saw) said,
“The most superior way of asking for forgiveness from Allah is: `Allahumma Anta Rabbi, la ilaha illa Anta, khalaqtani wa ana `abduka, wa ana `ala `ahdika wa wa`dika mastata`tu, a`udhu bika min sharri ma sana`tu, abu’u laka bini`matika `alayya, wa abu’u bidhanbi faghfir li, fa innahu la yaghfirudh-dhunuba illa Anta. ‘O Allaah, You are my Lord, none has the right to be worshipped except You, You created me and I am You servant and I abide to Your covenant and promise as best I can, I take refuge in You from the evil of which I committed. I acknowledge Your favour upon me and I acknowledge my sin, so forgive me, for verily none can forgive sin except You.’ Conditions of tawba: 1. That he must stop the sin. 2. Have a sense of remorse. 3. Be determined never to repeat again. 4. And he must correct the wrong act if he is able.

the reason i ask is cuz ive felt a decline
in my spirituality...also, bad habits like swearing are coming back. So i just got a little concerned. Pray that i can heal myself of these sicknesses cuz the last thing i want is a roadblock in my path to becoming a wali. Thanks
