Medical "Doctors" are given the title "Doctor", as an honorary title. i.e. They are "fake" Doctors.

Glasgow is correct about the academic hierarchy, but don't worry, MDs generally make more money.

I dont know if theres a formalized hierarchy of academic degrees anywhere, but as I understood it there are alot of degrees that are equivalent to the PhD which would obviously include MD, DO, DDS, JD, DVM, DPM, and all the other professional doctorates, but also other research degrees like DrPH and so on. MD or DO is the highest degree a person can achieve in medicine. You cant get a PhD in orthopedic surgery or internal medicine or whatever.
Is the academic hierarchy a self-declared one created by PhD-holders?

Nah, it's not like that. It's usually part of the graduation handbook because it determines the practices for the conferral of degrees such as the order at graduation, who can hood people getting doctorates etc (which is why academics care about it since that's who confers degrees). It might also be in something like a government salary handbook because the degree determines things like salary and GS level. Other than these kinds of situations, I don't think it matters that much and doubt that most people give it much thought.
