I worked with a resident who would write his daily notes on patients without seeing them. Like he'd say "Mr. X is fine today" when in fact Mr. X had spent the last night throwing up and running fevers. I got so pissed off when he missed a potentially disasterous complication in one of my patients that I reported him. The third year med student shouldn't have to tell the resident that his patient who had surgery the day before is blowing up like a water ballon!

And then I had this gem of a patient on psych. I hate personality disorders.
