If you graduate from an American medschool, your chances of matching into a canadian residency are quite good, and it happens all the time. The Canadian residency match occurs before the American one, and if you match into a Canadian residency program, you are automatically withdrawn from the US match. US and Canadian schools are both accredited by the same accrediting body - the LCME, and US grads are not considered foreign medical grads when applying to residency programs in Canada.
However, if you go anywhere else, it'll be a very tough road if you want to come back to CANADA. The US is a different story, carribean grads tend to do better there, especially in primary care programs. But because there's so few residency positions in Canada to begin with, and the Canadian government has shown no interest in opening up more residency spots, foreign grads have a very tough time matching into a residency program. I believe last year's match rate for foreign grads was like 10%.
There are so many foreign medical grads in Canada who are either without jobs, or doing things like telemarketing, or driving taxis. I'm not trying to discourage you, but this is the reality. Don't let the 'doctor shortage' fool you.
