Maybe Allah (swt) has a different plan for you. Things change all the time in programs, especially surgery. At my husband's graduation, only he and one other person were there from internship year through the end. Everyone else moved around somehow (research, went into anesthesia, went to a different program after getting married etc). So, you never know what might happen. Alhumdulillah, you got a surgery internship so you won't have to repeat the year. It sounds like the program director is willing to help you, so concentrate on making a good impression next year and don't worry so much about what kind of spot you have, but that you have a spot in a good place.
