It's impossible to say from the info we have what precisely was wrong with this woman, but she clearly had very severe psychopathology and the boyfriend was either mentally impaired or had psychiatric problems himself for not recognizing her behaviour sooner.
I'm guessing she was suffering either from a severe anxiety disorder (such as agoraphobia) or was frankly psychotic.
